Abstract

Background: Ethical dilemmas in surgical practice can significantly impact patient care, surgeon decision-making, and healthcare policies. These dilemmas are complex, multifaceted, and vary across different surgical specialties.
Objective: To identify and analyze the ethical dilemmas encountered by surgeons, explore the decision-making approaches, and propose strategies to address these challenges effectively.
Methods: A comprehensive narrative review was conducted using PubMed, Scopus, and Google Scholar databases. Studies published in peer-reviewed journals focusing on surgical ethics, informed consent, surgical innovation, and palliative care were included. A thematic analysis was performed to categorize key findings. Inclusion criteria consisted of studies addressing ethical issues in surgical contexts, and those discussing decision-making frameworks. Data were extracted and quality-assessed using the SANRA checklist, focusing on methodological rigor and ethical relevance.
Results: Out of 120 studies screened, 50 met inclusion criteria. Key themes identified included ethical challenges in surgical innovation (30%), decision-making in palliative care (20%), legal dilemmas (25%), and conflicts between patient autonomy and societal interests (25%). Ethical issues were most prevalent in the fields of pediatric surgery (15%), plastic surgery (10%), and global surgery (20%).
Conclusion: This review highlighted the need for structured ethical training, legal frameworks, and support for surgeons facing complex dilemmas to enhance patient care and professional integrity.
